I have faced a strange problem with my integrity B.11.23 server.I was not able to take putty session. After that i found out high cpu usage. When i checked logfile found vmunix error- vmunix: read_efi_hdr: physio failed.
This is BCV server used for taking backup of productions. Our backup team uses script which deactivate/activate VGs.That u can find in syslog.
Please suggest on this...
Thanks in advance..

SolutionOn the patch database, I see ├Г patch PHCO_38830
which is related with this sort of error.
Perhaps it could help.
If it's not good. Better to call the support.
For me, this message it's like if the system does not read a part of the efi, like an Efi partition error or idisk problem.
Try to see with efi_ls the efi partition s1 and with idisk the partition table.
HTH.

Re: vmunix error
Usually, this kind of error is related to the Boot Disks for the IA family of server. You may have to check for any boot disk failures. Please check if your boot disks are good.

Re: vmunix error
I hope you have mirrored disk for vg00, boot with other disk and replace the disk.
for the time being you can try to recreate the EFI with mkboot utility.
