Driver / controller error

Cannot get the tape to function properly. The green light is on implying everything is okay. However, when I load a tape it ejects it immediately. Concurrently, W2000 states in event viewer there is a controller error although I have tried it with both Adaptec 29160 and 29160n controllers with the newest drivers and control panel -system devices state the drive is functioning properly. TSMC reports there is a hardware error 0x0, device not ready when I try to test it. Any help would be greatly appreciated.
