seems to be a IBM branded Drive, with IBM FW.
If generic 4mm tape drivers and the suggested ones do not works , I think the best way to solve is to ask IBM for the right one, and also for a diagnostic tool (it may be the drive is not working properly).
Also be sure to use the right cartridges,
and if you are using NTBackup, be sure to stop all Backup Software services before to run the test,
If you are running a test with a Backup SW, be sure to stop removable storage before to work with it
