Re: HP DLT 1V
See http://www.hp.com/products1/storage/products/storagemedia/tape_dlt/faqs.html
Check how you store tapes. Overheating? Magnetic fields? Do not leave tape in the drive if not in use if possible.
Check drive and tapes with HP LTT http://h18006.www1.hp.com/products/storageworks/ltt/index.html

Re: HP DLT 1V
It's very unusual for DLT media to fail. I think I have had four or five bad tapes out of hundreds in over 7 years of daily use. I do assume that you take reasonable care to store the tapes in a temperature/humidity controlled environment. They are also very susceptible to physical damage causing by dropping the cartridge. In your case, I suspect that the tape is not bad at all but rather the software is detecting prior sessions and not allowing you to overwrite them.
If it ain't broke, I can fix that.
