HP Ultrium 2SCSI_1 could not be opened("Device could not be accessed")

we are using MSL6000 library connected with two rx4000 servers(RAC)the primary server couldn't access the drive, I run the ioscan :

H/W state: No H/W what is the problem??? It was working befor three days!!!
