L&TT Device Not Accessible - Ultrium 4 on Win2008 R2
I have a Proliant DL380 G5 using an external Ultrium 4 1840 SCSI drive.
When running Win 2003 server 32bit, everything was fine.
I have upgraded to Win 2008 R2 x64, installed the latest HP Ultrium 4 firmware and drivers and now the device fails to backup.
The L&TT reports 'Device class name '\\.\Tape0' file open failure. Looking at the LSI_SCSI Adapter it reports 'Device not accessible - cannot determine class driver, or open exclusively'.
Device Manager reports no error.
I've also plugged in another Ultrium 4 device and cable but get the same results.

This is the key in most cases. Check if the backup software or the windows RSM is running (LTT should check this too IMHO).

As suggested by Torsten, this could be because of the backup application has opened the drive already. If any other applciation has opened the drive, L&TT will not be able to communicate to the drive.

I had the same problem, very irratating.
Make sure you close the program or stop all the services related to the program, then try opening say L&TT and it will open fine, if not, there is probably something open using the drive.
Best best if unsure, restart the PC and don't open any programs and try opening L&TT first.
