Tape error message

I get an error message every other week with my tape drive. I have replaced the SCSI cable and the tape unit in the past couple of months but I still keep gettin errors. I am going to attach the message so you can take a look at and figure out what is happening.
Yes I have gone through all the steps that are listed and it still gives me an error.

If you replaced terminators and SCSI cables and still with the same problem, I'd try replacing the SCSI card and drive.
