can't take backup on sco unixware 7.1.3

dear all;
i have Fujitsu primergy TX200 server with LSI mega raid card. i'm trying to bakup my files to the DAT72 dat drive. for the first time after reboot the following message appear "ksh: /dev/rmt/ctape1: cannot create [I/O error] "
after that it working ok and complete the backup but if i tried to restor it it halt after awhile with the following message
even after install the latest maintenance pack and the latest driver i got from both lsilogic and sco but nothing changed.
please help

sorry the error message when restore is "ux:cpio:HALT: cannot read from device"

Hi Mahmoud

Clean the tape head using cleaning tape and insert new media and then try backup and restore.

i already do that; i also changed both the drive and the tapes !
