SolutionGeneral causes can be the driver or the timing settings in the driver (or in registry for the driver).
Considerations to take into account is the type of storage (SAN ?) and cabling/GBICs/switches/zoning.
I can see it is a Emulex card so looking into the driver version is a good starting point. Also the type of storage may dictate settings in the driver.
Driver-wise there is also the new Storport driver-model to consider

This is different and suppposedly better than the old miniport or fullport drivers.
