SolutionI beleive this is a timing issue with the vMedia boot device (which I'm guessing you're using) - apparently its going to be fixed in some future version of Ignire, but for the time being I seem to recall the work-around is simply to re-select the source device, which you can see from your output is 0/0/2/2.0x2 or index item 9.
