Virtual Connect SAN Fabrics - Server Connections Unavailable

I am having an issue where we are not getting any SAN connectivity on new blades we install in the chassis. If I look under SAN Fabrics, the Server Connections, the new blades show their HBA port as unavailable, even though the port does log into the fabric. The only way we have found to correct this problem is to reboot the Virtual Connect FC module and then the HBA port turns to OK. We are running HP BL495G6's with LPe1105's. The virtual connect modules are 8GB-24 ports running 2.32.

This might not be the problem.... but check the VC-SAN firmware version.
2.32 is the Ethernet version. The SAN version is included in the fw bundle and is 1.4 (or higher).
Often the ethernet one will upgrade and the SAN one will not as FTP is present on the machine you are using to update this.
Try to use the Virtual Connect firmware update utility from a desktop rather than a server if it will not update.

The latest version for the 8GB is 1.01. I ran the firmware utility and here is a screenshot of the results
