Solutionround robin with IOPS=1
There is a bug in 4.0 and 4.0U1 that IOPS does not stick on a reboot, but that is fixed in the just-released U2:
http://www.vmware.com/support/vsphere4/doc/vsp_esx40_u2_rel_notes.html
(just search for 'iops')

Re: ESX 4 on EVA6400
Agree with comments above, we faced big issue with multipath setting aftr upgrading ESX 3.5 to 4u1. See best practices on http://h20195.www2.hp.com/v2/GetPDF.aspx/4AA1-2185ENW.pdf , do not forget set vdisks to failover/failback, use RR with IOPS=1, works great.

Re: ESX 4 on EVA6400
Check the following link -
http://www.vmware.com/pdf/vsphere4/r40/vsp_40_mscs.pdf
-> Setup for Failover Clustering andMicrosoft Cluster Service
ESX 4.0, ESXi 4.0, vCenter Server 4.0
Hope this helps.

Re: ESX 4 on EVA6400
"With native multipathing (NMP), clustering is not supported when the path policy is set to round robin."
Now, I don't know many customers who even have played with MSCS on ESX.
