ML 570 G2, Blue screed after replacing the faulty HDD

Hello,

I have ML 570 G2 with 2 arrays as you can see from the report i attached, one of the HDD in array B failed and whenever i try to replace it hangs and blue screen pop up.

tried to replace the array controller which is 6400, updated the firmware as well as 6400 driver from psp and still getting the same problem.

could you have a look on ADU i have attached please and guess :)
