NBX100 only Discovers 1 port on 3C10117 ATC

Does anyone have any experience installing one of these cards, or any other ATC in an NBX100? I found one in our server room and would love to get rid of the 3 ATAs that are crammed into my phone cabinet, but when I plug in the card and do an Auto-Discovery it only finds 1 port, I'm able to plug in and use Port 1 on the card but 2, 3, & 4 are all dead in the water, no tone and no S1 light when the phone is off-hook.
