I am using HP-UX 10.20 K-class server.
I am daily receiving following two messages(in brief) for a disk in STM logs:
1st message:

Device Path: 10/0.6.0
Product: SCSI Disk
SCSI Status...: CHECK CONDITION (0x02)
Indicates that a contingent allegiance condition has occurred. Any error, exception, or abnormal condition that causes sense data to be set will produce the CHECK CONDITION status.
Sense Key 0x03, MEDIUM ERROR, indicates that the command terminated with a nonrecovered error condition that was probably caused by a flaw in the medium or an error in the recorded data. This sense key may also be returned if the device is unable to distinguish between a flaw in the medium and a specific hardware failure (sense key 0x04).
The combination of Additional Sense Code and Sense Qualifier (0x0C 0x02) indicates: Write error with failure of auto reallocation.

2nd message:
----------------------------------------------
SCSI Status...: GOOD (0x00)
Indicates that the target has successfully completed the command.
----------------------------------------------
Are the above messages points to any exception?.
This disk is a part of root VG and extents of /, swap, /opt, /usr, /var & /stand spread on this disk.
Any kind of comments are welcomed.

SolutionThe 1st message indicates that the disk is starting to produce errors and should be replaced.
The 2nd message suggests that the SCSI card has been tested without any errors being produced. (don't worry)
If you have your internal disks mirrored, you shouldn't have a problem, get the drive replaced. If you have not got this mirrored, make sure you have an up to date ignite tape handy. To create an ignite tape:
# /opt/ignite/bin/make_tape_recovery -x inc=entire=vg00 -I -v -a /dev/rmt/0mn
If you dont have the latest ignite software you can get it from here.

I am sorry to be the bearer of bad news but 10/0.6.0 is probably your boot disc. Replacing it may require re-installing the o/s.

by the error message, it shows that first disk has gone bad. second disk is ok.
