RX7620 system crash when attempting to install HP-UX

I have recently obtained a second hand RX7620 Itanium Server for development purposes, however when I try and boot the HP-UX installation media from DVD it panics when loading the kernel.

I have tried both HP-UX 11i Version 3 media from September 2008 and HP-UX 11i Version 2 media from June 2008 but get the same crash from either. I have also updated the firmware to the latest versions.

What kind of card is in 1/0/12/ (slot 3 - right half)?

Thanks for the hint. It turns out the crash is caused by a PS2 -> USB adapter I had plugged in to connect a keyboard and mouse. Unplugging this fixes the crash
