Have you checked everything from the physical side?
Output of port stats? Any CRC errors? Is the fiber cable inserted correctly? (Are the transmit and recieve end of the cable flipped?)
If you have iLO or OneView, you should get a more detailed error message. Check the IML logs. Let me know if any of this helps.

bind interface Ten-GigabitEthernet2/0/12
Ten-GigabitEthernet1/0/12 goes to Proliant NIC1 and Ten-GigabitEthernet2/0/12 to Proliant NIC2
3Par has 2 interfaces on VSAN10 and 2 on VSAN20, but other Proliants have no similar issue. Any ideas?

But in my first post, first picture Network Ports is physical ports, right? I think they are ok, just FCoE ports are not forming. Integrated management log shows nothing with an issue.
I connected to host and checked that both links are full duplex 10000mbps.
