Just to clarify; SLOT 4 is on the PCI I/O module. This may or may not be Bus 4, you'll need to post an ioscan -fnC disk so we can see how the system is configured.

There may be a problem with c4t0d0 or c4t1d0, but most likely with the SCSI controller, the cable or the terminator. I would check cable and terminator first.
