KIndly attach the output of vg_stg_us04.
vgdisplay -v vg_stg_us04
This disk is belong to this vg.

The are 2 incidents:
1.- SCSI: Parity error -- lbolt: 21360829, dev: 1f020000
LVM: VG 64 0x000000: PVLink 31 0x021000 Recovered.
2.- LVM: Failed to automatically resync PV 1f021000 error: 5
0/2/1/0.20.133.128.0.10.2 sdisk
0/2/1/0.20.133.128.0.10.3 sdisk
Perhaps BCV establish without deactivate VG

/dev/dsk/c21t10d2
/dev/dsk/c21t10d3

While taking ├в make_tape_recovery├в i have found it hangs after progressing below message. Please take necessary action at your earliest.
# make_tape_recovery -x inc_entire=vg00 -I -v -a /dev/rmt/0mn

System Recovery Archive
** 1 - The Volume Group or Filesystem is Partially included in the
System Recovery Archive
** 2 - The Volume Group or Filesystem is Fully included in the
System Recovery Archive
