Since the Array State=No Map, I cannot create any LUNs.
Can anyone help?

That should show something like this ..

Have you try to reset ARMServer daemon.

it's saying a "field replacement unit" need to be replaced. I don;t know what's 129 is but I'm guessing it's controller X. Before you jump into any conclusion, I would try a few things first ..
1- Reset ARMServer
2- Physically examine controller X, it should have a constant green LED if in working order.
3- Hardreset your 12H (only if you have the downtime window)
If all the above does not help I'll more like to draw a conclusion that controller X needs to be replaced.

Press Menu, then "+" until you see "Disk Changes" and then press Enter. Somewhere in there there should be an option that says "Include Disk", choose that one to perform the include operation of this disk into the array.

and I confirmed that but it said No Disks Included

Do you have LUN 0 created on the 12h?
Software can tell you incorrect things because of many reasons, but 12h's front panel - never. If it says 'ready' then it's really ok and there're problems with software. Controller X in unknown state can be because it is simply no connected to this host

I think this problem is to do with the fact that these disks were taken from another 12H Array, since:

Information for disk in slot A1:

etc etc.. for 4 more disks

The Problem:
No Address Table
I think this means data maps are lost. (data on the disks is not needed).
I've tried formatting the array, but still:

Ok, this is what you should do, first unplugged all the disk modules out from the slots. Now plug in just one disk to slot A1. Follow the procedure I gave ..
Menu->DiskChanges->IncludeDisk and I think it'll either detect that you got a disk inserted into A1 or it'll prompt for you to choose the slot number. After you've included the disk in, run arraydsp again and see if the status change. It may take a while for the array to process and update its information, so be patient. If this goes well, do the same for the rest of the disks.

You were messing around... so the AutoRAID is now messing around with you.
Most probably, you removed a number of disks and put in some disks from another AutoRAID. Or else, you replaced a controller from another AutoRAID. In order to solve this, you need to either return the disks you replaced, or return the controller you replaced.

I did not remove the controller only the disks!
I will see if I can format the array from the Front Panel
Cnrl Changes> Format Array>Clear Data>Confirm
Tried formatting with arrayfmt but that did not work..

in one of your replies you stated 'tried formatting the array but received "no maps"'. Next time please be more specific how did you do such things.
I thought you were using control panel (in most cases it should be used) because ARMServer clearly was telling incorrect information and thus should not be used for managing 12h
