Check syslog.log and dmesg for errors as well.

Also, make sure your device name didnt change... and are you not using the no rewind option?

I hate it when that happens...
I once was rlogin'd over to another box and inadvertenly rebooted the wrong one... but heh luckily it was during my maintenance and didnt cause too much headache...
