Damage Hard Disk

I have Windows server 2008 installed on DL 320 G6 server contain 4 HDD ( each one 500 GB ) and configured as RAID 5.

two days ago became the color of one of the disk red and after that lost some data.

 

- How to make sure if the disk damage or not ?

- There is any procedure to replace disk in RAID 5 mode ? or I can replace normally by plug-in  and plug-out ?
