I am new to handling servers but good at troubleshooting PCs. I have an HP Proliant Desktop Server in my University Department which acts as a domain server. It shows a
"slot-2 HP Smart Array P420 controller
1783-slot 2 Drive Array Controller Failure!
[self-test failure (cmd=0h, err=00h, lockup=0DEAD:DEADh)]"
This server has two array controllers
1. HP dynamic smart array B120i RAID controller (Embedded)
2. HP smart Array P420 Controller (PCI slot 2)
Please Tell me a way to resolve this issue??

Hello Dark_Force,
Appears that a lockup has occurred on the P420 controller.
you havent mentioned the server model here though..
You can upgrade the controller firmware and confirm the status:

** it is always good the update the last FW version as a part of the common TS steps.
also , because this CTRL is an additional card and not onboard, you can reset it, or even test it in another slot - so you will be able to test the SB (the slot) as well.
And as a 3th option, I`ll say - if the CTRL has a cache module - please remove it and then test the CTRL without it - if the cache is failed, that may cause such initialization problem as well.

Hi sir
Kindly check to which controllers HDDs are connected to B120i or P420 controller
- Disable the Un-used controller in the BIOS
Issue : 1783-Slot X Drive Array Controller Failure
Cause
The controller firmware is outdated.
The cache module is not installed correctly.
The controller is not installed correctly.
The controller has failed.
Action Plan
Reseat the cache module.
Reseat the controller in the PCI slot.
Update the controller to the latest firmware version.
If the issue persists, replace the controller

The HP smart Array P420 Controller (PCI slot 2) is not listed in the HP smart storage administrator.

Solution
Dear Dark_Force,
OK, that is because of the self test error - then the FW update will not help us here.
Please try to remove the cache (if any) and test.
- if it works without cache - the cache should be replaced.
- if the same - try to move to another slot if possible - to isolate a possible SB issue.
If nothing helps - the CTRL needs to be replaced.

[Inconsistent volume count (B)]
[Unexpected hardware revision – hardware rework needed]
[Unsupported Flash ROM type installed]
[iLO communication mechanism self-test error]
[PROGRAM BUG! Insufficient padding bytes (cmd=##h)]
[Incorrect EEPROM type]
[Init failure (cmd=##h, err=##h)]
[Command failure (cmd=##h, err=##h)]
[Self-test failure (ErrCode=####h)]
[I2C NVRAM reconfiguration failure]
[PCI bridge missing]
[PCI bridge disabled; check System ROM version]
[PDPI not found]
[PDPI disabled; check System ROM version]
[Board ID not programmed]
Cause
The cache module is not installed correctly.
The controller is not installed correctly.
The controller firmware is outdated.
The controller has failed.
Action
Reseat the cache module.
Reseat the controller in the PCI slot.
Update the controller to the latest firmware version.

I removed the Cache module and started the HP smart storage administrator. It showed the Array controller with an error obviously that "cache module is not present so controller has been disabled"
Then I reinserted the cache module back and started the server, It now ran without any error. But it said the cache module has been disabled.
My problem is now solved

Similar scenario in my stack, but the problem was that the storage bay was not receiving enough power for all the LFF drives. We stumbled onto this when going through and reseeding all the Hot-swappable LFFs. We unseeded all of them, then reseeded them one at a time. During this process we discovered that the last three caused a shutdown of the system. It didnt' matter which LFF or which bay the last LFFs went into (have a few open ones), once they were plugged in, the system shut down and upon reboot had the error.
