Replacing and rebuilding a problem disk on a Proliant DL380 G1

We have been having some problems on a Compaq Proliant DL380 G1 with one of the disks. The light on disk 0 turned orange once but after reboot it looks like it has been reset. It is in a rack and I have not always been informed on reboot, so I'm not sure if it had an orange light before.
The disk controller is a SmartArray. I've been informed that 0+1 RAID is being used, and the operating system is FreeBSD 4.8.

What can I use to verify that the disk 0 is the problem disk ?
If I can verify that it is disk 0, can I simply do a hot swap and the SmartArray system will automatically resync the new disk ?

Run Array diagnostic Utility from smart start CD 5.5
If some problem found
Pull drive out when system down if you not sure about FreeBSD and add new one

Where can I get the SmartStart CD 5.5 ?
The original pack for this machine seems to have almost everything except the SmartSuite CD.
