Solutionnot sure what is occuring here
from the log I still see various hosts accessing at the same time the same device.
Would be good for a troubleshooting point of view to reduce the hosts to the minimum, zoning them out, and creating a dedicated map, and see if reducing the interferences you solve that situation.
Also there are some hosts that are Polling the drives, these hosts should be cheked and all the recommendation in the EBSDesignGuide should be applied
attached is the extract of the log from the LTO showing the time of the error, do not see hw issue here.

Just wanted to add, that this post also helped me.
I had problems with a taperobot that made the same error, when trying to change the medium.
This started after I moved the robot to another location.
After the move I had the option to connect both fiber fabrics to each drive, so I did...
The solution was to remove one of the fibers from the tapedrives, so there was only 1 path to each drive again.
The errors then stopped :-)
