Ultrium 232 Refusing Tapes

Attached is the health report from HP L&TT. Repeated Clicking and eventual ejection after a couple of minutes. Backup Exec 11d recently had issues regarding it's services and configuration, however it seems coincidence as this appears to be independent of software. Ran through the troubleshooting on HP site to no avail. The tape LED flashes after ejection, and the ready LED flashes up to the point of ejection.
