HPUX B.11.11 - Boot disk Issue

Hello please help me on this boot disk issue.

we have mirrored boot disks (pri & alt).
The dmesg shows I/O error - powerfailed on primary disk. Also pvdisplay shows stale disk.
However ioscan shows the disk CLAIMED.

I rebooted the machine using ALT path (2nd boot disk) and its not booting up (we never boot up using the ALT disk previously).

So, I booted again using PRI boot and booted success. Now I am not getting I/O - powerfailed. but i can see still stale disk.

I am going to replace the PRI disk. Also, I have a good ignite backup (taken before the i/o error).

question is, how can recover this. Also, how can I re-establish the mirror with 2nd disk?
