The dmesg output mentions HSV200 and EVA, which means you're connecting to an EVA4000 or EVA6000.

I had a trouble like this, a think that the problem is the driver of your fibre channel HBA.
We upgraded the linux kernel of one server ( Red Hat 3 up 8 to Red Hat 3 Up 9(I know is a little old but..), and we have to upgrade the HBA drivers ( by the way it is a Qlogic), everything went fine until we test the MSL6000 and it was not shown whith the command cat /proc/scsi/scsi and it was not posible to backup servers.
So we back to the old kernel with the old HBA drivers. :-(
I have requested HP support.
