Physical Entity Failed: Hardware error:/legacy/path/of/disk/ DEVICE HP HSV300

HI

Getting alerts in our monitoring  system.

identified NO_HW disks from ioscan -fnC disk

did rmsf /dev/dsk/cxtxdx and /dev/rdsk/cxtxdx

did ioscan and insf

the NO_HW disk  appear again

 

ioscan -m lun shows:

 

vmhost # ioscan -m lun | grep NO_HW
disk     96  64000/0xfa00/0x2e   esdisk  NO_HW       DEVICE       offline  HP      HSV300
disk    101  64000/0xfa00/0x2f   esdisk  NO_HW       DEVICE       offline  HP      HSV300
vmhost #

 

these are not seen in evainfo -a output.

 

any idea how to resolve this please?
