RHEL 3 is fairly old, and many kernel bugs have been patched during its lifetime. Is your system up to date?
Please show us the output of the following commands:
cat /etc/redhat-release
uname -a
cat /proc/scsi/qla2300/?
These will allow us to identify the update level of RHEL3, kernel version and QLogic driver version (assuming I've guessed your HBA type correctly).

So.. your driver version is 7.05.00-RH1.
The latest driver version for RHEL 3 seems to be 7.07.05.08. It might be good to update. This driver version was released in mid-2008, so it certainly should not be "too fresh to trust" any more.
