4si scsi card not responding

I am getting this error message:
IRMD[Severe]: Adapter 0/10/0/1 is NOT RESPONDING

Rebooting seems to fix the problem. But after few minutes of usage, recieved the same error again. I've replaced the scsi cable and the 4si scsi card. The problem still persist. Any idea? Thanks.

Installing this patch may solve your issue. Read the patch description if interested:

PHSS_27097 for HP-UX 11.00
PHSS_27098 for HP-UX 11.11
