Looks like the system is having trouble initiating any I/O onthe SCSI controller. Could be a hardware problem on one of the scsi devices or the scsi controller itself.

Also, make sure all the connections are good and that the terminator is also connected.

Scsi controller - on bus 4 and 5
Do and ioscan -fnc bc

to see hardware addresses.

Check (docid=KBRC00001951). It'll give you some idea what it meant. To me it looks like your scsi controller is failing.

This can be:
a patch issue
a SCSI error. Check the devices with stm tools:
# stm
learn unix ..

Do u ahve a Jamica Boxconencted tot he amchine , it looks etiher there ia power porblem or on of the disk in the enclosure is not good.
