Error in Syslog
I find this error in syslog:
login: load_modules: can not open module /usr/lib/security/libpam_unix.1
login: load_modules: pam_sm_acct_mgmt() missing
Can some body tell me what it is?

reduce the value tcp_time_wait_interval
/TIME_WAIT state- duration/:
ndd -set /dev/tcp tcp_time_wait_interval 30000

If you look in the doc I linked to your question
In some cases, increasing maxuprc, nproc, and the amount of swap on the remote
system may help. You need to eliminate these errors before investigating the
PAM error messages. In most cases, these errors might have existed prior to
installing the r-commands patch.

Swap in general should total 1.0 to 2.0 times physical memory until you get to the really HUGE machines.
You are clearly overloading your kernel settings.
