did you try to rewind?
# mt -f /dev/rmt/
then perform frecover.
by the way, do you mind sending the frecover command you have executed? also, what is the latest patch on frecover?
# what /usr/sbin/frecover

Have you made any changes to the hardware recently? This error can be seen if the device cannot be opened or identified.
You really need to look at the script that you've run to see what other commands are run.
