High CPU utilization with vxfsd running as top process

Yesterday we had 1 of our v-class servers running 11.0 in a MCSG enviroment TOC. Before it went down and rebooted vxfsd was constantly running as the top process. After the server went down we looked in crash logs, tombstones, syslog, and shutdownlog, but could not find any information about the crash. Does anyone have any idea what maybe caused this or how to prevent it from happening again? Also after the load average had been around 3.0 or grater vhand started jumping up as well. We have 7 cpu's and 6 gb of mem. Thanks for the help.

How about doing:
1.
#crashutil -v COREFILE /var/adm/crash/crash.0
/tmp/corefile /tmp/kernfile
after system TOC?

2.
#cd /tmp

3.
#adb -k kernfile corefile
or: #dmesg COREFILE kernfile

There are no corefiles or kernfiles in those directories. I have not been able to find any sort of crash file.

This can also be HW with memory failure

It seems that we only have 2 of the patches you listed, PHKL-18543 and PHKL-20016. I'm looking into the other patches now. Also, there is nothing in the shutdownlog for the TOC. I'm going to reboot the server and see what happens when it comes back up.
