Instance Number exceeded
I have an issue on a large database server with a great deal of SAN disks. It seems that the HBA's were reconfigured several times and the disks were moved to different ports for performance reasons. Now we are in a situation where we cannot add any new disk instance numbers due to an error from the kernel indicating we have exceeded our fcpdev instance number of 255. Will ioinit -c in conjunction with removal of /etc/ioconfig and /stand/ioconfig solve this problem?

Yes, you are headed in the right direction. You probably have a number of inactive instance numbers from the various changes that were made to your san over the years.
Here is a procedure to deal with that issue.
Do a make_tape_recovery backup before you begin, this procedure is destructive.
Any applications that use raw disk will have to be reconfigured.
There is also a good procedure on this in Keenen's book HP CSA.
