1. user account is in deactive state
2. user use incorrent password
3. user connect to wrong server

How are the users logging in? Local console, X-Session, Telnet, SSH? Have you checked the status of the users account through sam?

If after password changing you are not able to login to that user , try this.
# sam
Reactivate the user , if it us deactivated.
Also you can check with pwck ,
To check inconsistencies with the Protected Password database.
