what do u guys think can be the problem...

Mistyped the confirmation entry.
Is the passwd file already open, by someone else, or an old process left the file open?

The password file is opened and locked by vipw command. Look for it by
ps -ef | grep vipw
or look for vipw lock file /etc/.pwd.lock
