Possibilities that come to mind:
Network issues
Agent not running on client to be backed up
Tape issues
Tape drive issues
Backup server issues

Veritas 3.4 uses /etc/hosts networking no matter what settings you have in nsswtich.conf
We're testing an upgrade to 5.0 now and I'm sure thats going to be fun.

Tape library offline
Bad media
Client agent not running
backup won't start outside window
only partial backup due to files being open
bad network.

We did a test last night disabling survey, everything went on OK. Now to find out if there's a patch/fix ...
