c) scsi terminator has failed/been removed
d) scsi cable is faulty
e) some other problem in daisy chain

Re: SCSI ERROR
The message means a SCSI device status could not be known completly through SCSI commands internally. Thats why it is referred as "partially opened". This SCSI device could be a LUN created on disk array.
The disk array who create a temporary LUNs e.g for split BCVs may be cause of this message in syslog.
If you don't have problem with the SCSI device in your setup, then this message could be considred as "informative". SCSI I/O Cummulative patch ( latest PHKL_32090 for 11.11) could be applied to avoid those "informative" messages. Thereafter if SCSI level debugging is required , a flag could be set producing such messages in syslog.
If there is genuine problem with a SCSI device, then it should be fixed depending on the situation, may be at array side and hardware level etc.
