Solution1) replace the failed disk
2) recover it's contents

First of all you must plan at all times for a failure as it will happen.
1. Mirror your os disks.
2. Mirror your data disks.
3. Use ignite.
4. Backup everything on a regular basis.
5. Backup your data as often as possible - min onece a day.
6. Test your backups - are they valid and can you recover files.
Disk procedures depend on the structure of your disks.

The steps to recovering a failed disk vary depending on whether or not the disk is hot-swappable, mirrored, is a boot disk, etc.
The HP-UX Software Recovery Handbook's chapter on LVM provides an excellent guide to these procedures:
