troubleshooting a printing problem

I am having a problem with printing a particular quiz file to a hp 2564b printer in hp-ux 11 and I was hoping someone had an idea. I have included the quiz file. This file prints inventory tags. When you print it, the first 4 tags print correctly. After that, it will start the 5th tag 1/2" too low. I have tried this on several printers with the same result, so I figure it must be a problem with the quiz file but why would it work for the first 4 and not the 5th?
