The only time I've seen this is when a disk is going bad. If it were me, I'd be calling HP ASAP! :-)

You may have a disk hardware failure. The error number is 126 which may relate to disk array controller problems or a possible patch or LVM setup problem. There are a number of different conditions that lead to the 126 error so you'll need HP's help to find the real problem.
