Help regarding excel parsing with PERL

Hi I want to draw a datamodel,So i want to parse an excel sheet like "input.xls"(it's attached).I have parsed the excel by PERL but my actual requirement is a bit tough.

Here are the relation between parent table & child table.Beside the table names the column names are written which will connect both the tables(the column name can be anything).My ultimate aim is to create index for the tables using these column names.
I have attached both the input & output files for my requirements.Also attached the perl script what i have used for simple parsing.
So please help me providing a PERL script for this work to be done...

Hi:

I can't open your attachment. Also, you posted in both HP-UX and in Linux (here). I asked the moderators to delete the duplicate query in HP-UX. There is NO NEED for posting in both families.
