Please refer to the attachment and let me know steps to perform the action plan as per mentioned in the attachment, What's the resolution based on the case . thank you very much. Your effort shall be highly appreciated.

Any new hardware configuration? A new SCSI device added?

You should verify the status of the array and disks by using HP Array Configuration Utility. If you don't have it you can download it from the HP site.

You can try to boot in single user mode and verify the /var/log/messages file for more information. You can use:

kudzu -p

To view what is probed.
