RH5.4 with direct connection to MSA60
RH5.4x/DL380G6 direct connection to MSA60, fdisk does not see the disk/LUN?
Where should I start? other servers connected to MSA seeing the luns fine.

What model Smart Array controller have you installed in the server to attach the MSA60 too?
What's in the boot log or dmesg? do you see the cciss driver loading and identifying the storage?
