account administrator password reset error message

We lost the password for the account administrator. Cannot do updates or upgrades. Tried to reset and an error message comes up that state, "an error has occurred while wizard attempted to reset the password". There was no explanation as to what the error was, and no error number. Can anyone help?

What OS are you running?
Is it a PC or a server?
