You will want to read the Chapter entitled "Managing User Accounts" in the System Manager's Essentials manual:

As this and other associated documentation will lead you through the steps and the considerations involved with adding a user account, and with removing a user account.
Other materials in that manual will guide you through other tasks essential for an OpenVMS system manager.
Full OpenVMS system privileges are required.
Unfortunately for this discussion, the steps involved in the local Cognos Powerhouse application environment may well modify the steps required, or could conceivably even completely contravene the steps described in the manual.
