I may be jumping to conclusions. Did this disk work before you changed the SCSI cable? That was the assumption I was making.
If so, then it is probably due to the new cable and termination.
If the cable was changed to try to fix a recent disk problem on a disk for which you have no backup, then you have my sympathy.

Did you set (or even check) the SCSI address for the tape drive to be different from that of any other devices?
I have seen similar symptoms when there was a conflict...

I know I risk to seem to assume you do not know "trivial" facts, but those are always not so trivial until you find out.
SCSI accepts only ONE of each numbered addresses. On SCSI that old they are numbered 0 - 7.
Usually a Vax will be 6, an AXP 7, but I seem to recall having seen 6 as well.
Obvious (DKA100) you already have 1 assigned.
External SCSI devices have binary dip switches or dial wheels to set the number.
Now, check and/or set EACH one unique.
