Can you post the results of the ANAL/DISK/READ command (ideally as an text file attachment to your response to preserve formatting) then some futher assistance is possible.

Have you tried running the DFU VERIFY tool against this device as well? Are the errors on the DSA device name or the component member drives? If you do a SHOW ERROR do you see only the DSA device name listed or do you also see the members? What kind of drives are the members? If they are SCSI drives direct attached to the system you will also want VMS732_FIBRE_SCSI V7.0.
